Ri79-9,Health Benefits Cancellation and Suspension Confirmation

The following form can be submitted by people who are Civil Service Retirement System and Federal Employee Retirement System annuitants, former spouse annuitants, and survivor annuitants.  This form is used for canceling suspended enrollment in the Federal Employee Health Benefits Program. People using this form are advised to read the front and back of the form before submitting it.
