Arizona Adoption Forms

As a result of errors in an adopted child’s edited birth certificate or social security number, the adoptive parent/s might need to request for the opening of the child’s sealed records. To petition for the opening of the child’s file, the adoptive parent/s must submit a notarized copy of this form to the Superior Court of Arizona. If petition is approved, the Judge of the Superior Court can authorize the opening of the sealed records.

record due to an error such as a change in the adopted child’s birth certificate or social security and you 
are the adopted Parent(s) or adopted child, please complete the attached “Motion to Open an Adoption
