USA Boxing Athlete Physical Form

To be executed, this form must be accomplished and signed by both the athlete and a licensed physician. If the athlete is a minor, her/his guardian must also sign on the form. If the physician signed on the form s/he verifies that the athlete is physically fit for boxing. If the physician declares that athlete is physically unfit, the disqualifying medical reasons will be reflected in the form.
