Dear Customer:
Enclosed are Articles of Incorporation or  Application for Certificate of  Authority forms needed to file a domestic or foreign
corporation in the state of  Arkansas. We have also enclosed a Corporate Franchise  Tax information form that will assist us
in mailing your annual Corporate Franchise  Tax Report form directly to a contact person that you specify .The tax payment
and form are due May 1st in the calendar year following the year of incorporation or qualification and each May 1st there -
after.
Domestic corporations must submit duplicate articles completed and signed and the filing fee of $50.00. Foreign corpora -
tions seeking authorization to do business in  Arkansas must submit duplicate applications completed and signed, an orig-
inal certificate of existence from the domestic state of incorporation dated within thirty (30) days of our receipt of the filing,
and  the  filing  fee  of  $300.00.  No  application  will  be  processed  without  an  Arkansas  registered  agent  with  a  physical
Arkansas address.  At least one original of the articles or application must be submitted.
The completed tax contact information form should also be returned at the time of filing. If we do not receive the tax infor -
mation form, your annual Corporate Franchise  Tax Report form will be mailed to the registered agent of record.
Corporations wishing to file as nonprofit corporations or cooperative as\
sociations  DO NOTuse the enclosed forms. Please
