• An applicant must be 18 years of age or older and a resident of, or have a principal place of business in, Connecticut. 
• The entire application must

 be filled out completely and signed by a public official or a reputable 
business or professional person who must be unrelated to you and have personally known you for at least on year. 
• The Jurat on page 6 must be completed and sworn to before a notary public or other appropriate official.

order made payable to the “Secretary of the State” in the amount of $120.00 (non-refundable statutory appointment

NOTARY PUBLIC EXAMINATION 	
 	
This examination is divided into four sections and must be completed in the applicant’s own handwriting.  Each question of 
each section must be answered correctly before an applicant will be appointed a Notary Public.  The answers to all questions 
will be found in the Notary Public Manual	
.  The staff of the Office of the Secretary of the State will not assist you in the 
completion of this examination because it is a test of your basic understanding of the duties and responsibilities of the Notary 
Public in the State of Connecticut. 	
 
Section 1	 – Place an X in the box corresponding to either true or false for each of the following statements:
