In this worksheet, parent/s must indicate their plans for the minor children’s education, extra-curricular, sports, and other activities after the finalization of the divorce.
This form must be completed by the Defendant upon receipt of the copies of the divorce papers that the Petitioner filed with the court clerk’s office.
