Following the filing of the divorce papers, Petitioner must serve this summons on the Defendant. Upon receipt, Defendant must complete this form and return it to the Petitioner for filing.
This form functions as the divorcing spouses’ proof of income. Through the fulfillment and filing of this form, the court and both spouses learn about previous and future earnings. The information reflected in this form serves as the basis for the child support arrangement.
By submitting this form to the court clerk’s office, an individual learns if s/he is qualified to file for a divorce or not. Form is valid for use in the State of Wisconsin.
