This EPA-issued form allows a property seller to notify the prospective buyer regarding the possibility of the existence of lead-based paint on the surfaces of the housing unit. If the house was constructed before 1978, this form must be signed by all parties involved.

If a property was constructed before 1978, owner must serve this form to the tenant.
