Step 1	 - Fill-Out the 	Arkansas Name Change Petition	. The Petition must be 	
notarized.
Step 2	 - Make 3 copies.	
Step 3	 - File the Petition to the	 County Court Clerk's office	 in the County 	
where your live (See County List Below). You will need to file the name change 
petition with a cover letter that will be provided to you by the County Court 
Clerk. Pay the 	filing fee of $140	and you will be given a case number.	
Step 4	 - Upon showing up for your court hearing, you will need to fill out an 	
Arkansas Order for Name Change	.If approved, the Judge will authorize your 	
order for name change.
