Agreement to Modify Promissory Note Form

In the event that the borrower fails to pay on the promised date, the lender and the borrower can agree to alter the details of the original promissory note to accommodate the borrower’s financial situation. For the agreement to be binding, both parties must fulfill and sign this form.
